Once you click the “-“ button, you will see in the live preview that Formidable Forms has removed the option.

Next, you will need to create related questions for each option. This is because you want to set up conditional logic for when users choose one of the three options.

For the “Muscle Gain” option, you may be asked to know your food preferences.

Since there are dozens of options for food preferences, you can add a few of the more popular ones and add an “Other” field where users can type their preferences.

Once you do this, a “More” radio button will be added with a simple text box. If you haven’t already, click the “-” button for the “D” option.
